Guidelines

All clergy, paid parish/school personnel and all adult and youth middle and high school volunteers who have regular contact with students/minors, including coaches, parents/guardians, people assisting in classroom, and others must sign the Guidelines form before beginning their volunteer work or assignment.
The Guidelines form must be signed every year.

Please bring the receipt signed to the parish office in order to be processed.
